Job Description:
- Provide skilled nursing care to patients in their homes under the supervision of a registered nurse or physician.
- Perform comprehensive assessments of patients’ health status, develop individualized care plans, and implement appropriate nursing interventions.
- Administer medications, wound care, and other treatments as prescribed by the physician and in accordance with agency policies and procedures.
- Monitor patients’ progress, evaluate their response to treatment, and communicate effectively with the interdisciplinary team to ensure coordinated and holistic care.
- Educate patients and their families about disease management, medication administration, and self-care techniques to promote optimal health outcomes.
- Maintain accurate and up-to-date documentation of patient assessments, care provided, and other relevant information in the electronic medical record system.
Qualifications:
- Active Licensed Practical Nurse (LPN) license in the state of New Mexico.
- Graduate of an accredited practical nursing program.
- Minimum of one year of clinical experience, preferably in home health or a similar setting.
- Current CPR certification.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
