The Frailty Care Coordinator will work closely with the GPs and other primary care professionals to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers, and ensuring that their changing needs are addressed. They focus delivery of the comprehensive model to reflect local priorities, health inequalities or population health management risk stratification. They will be the first point of call for this identified cohort, proactively managing their needs, and coordinating the Multidisciplinary Team to meet them. They will provide holistic and person-centred care and support for people of all ages but with a particular focus on the care of older people/long term conditions and those living with Frailty as per the practice demographic. This role will assist practice staff in the provision of treatment, preventative care, health promotion and patient education. In addition, the successful candidate will be expected to support the administrative team. The role includes supporting digital initiatives and co-ordinating the patients journey through the health and social care system. The candidate will need to be an excellent communicator and be able to establish efficient monitoring and quality assurance systems.Main duties of the job1. To work with the GPs and other primary care professionals to identify and manage a caseload of patients including in attached Care Homes. 2. To work in partnership with the Social Prescribing Link Workers and other members of the PCN and MDT. 3. To support patients to utilise decision aids, help create personalised care and support plans, in line with best practice and with support from the wider team. 4. To provide help with the aid of digital tools for people and their carers across healthcare services.5.Proactively identify and work with a cohort of people to support their personalised care requirements, using decision aids.6. Bring together the identified care needs, and explore options to meet these into a single personalised care and support plan, in line with best practice 7. Take a link role within the Multidisciplinary/Frailty Team. Support the coordination and delivery of meetings and MDTs. 8. Coordinate and proactively manage the cohort, with clinical input.9. Take an administrative lead in queries and work relating to caseload. 11. Carry out baseline observations such as pulse oximetry, BP, temperature, pulse.12. When trained, undertake venepuncture.13. Undertake home visits when needed to identify patient care needs and provide support.   • Relevant qualification or experience in a similar role within health or social care Good standard of education Evidence of professional development within the health sector Educated to GCSE level or equivalent

Desirable

  • Degree Qualification in Health or Social care allied profession Completion of healthcare certificate

ExperienceEssential

  • Understanding of basic health and social care terminology Understanding of human needs physical, emotional, social Proven ability to recognise and manage risk Ability to understand budgetary requirements; ensure that packages are cost effective and value for money; and highlight inefficiencies Good interpersonal skills Ability to communicate confidently with staff of all levels and across organisations Ability to work well in a multi-disciplinary team Good time management and prioritisations skills, ability to work to strict deadlines Ability to work on own initiative without direct supervision, understanding where clinical input is needed Good IT skills and experience in the use of Microsoft Office products incl excel Co-ordination of services from a variety of organisations Communication and relationship building across teams

  • General Practice experience Familiarity with IT systems currently in use in health and social care settings Experience of health and social care assessments Demonstrate successful outcomes delivery within determined timeframes
