Do you want to make a real difference and drive the future of integrated responsive health and social care?We are seeking an experienced Operational Manager to lead our innovative service which includes our urgent community response (UCR), falls pick up services, seven day social care and virtual ward.JET is a seven-day service that operates from 08:00 to 20:30, including bank holidays. The staff team consists of qualified health and social care professionals and a range of support staff in order to support the needs of our Greenwich residents.This is an integrated role working across both health and social care that is jointly funded by the Royal Borough of Greenwich and Oxleas NHS Foundation Trust and is accountable to both organisations.The Royal Borough of Greenwich and Oxleas NHS Foundation Trust are dedicated to providing high-quality, responsive, and compassionate care to our community.The Joint Emergency Team provides a crucial role in delivering timely interventions that prevent hospital admission and support individuals to stay in their own homes providing a home first response. Alongside this we facilitate early discharges from hospital and function as a gateway to the integrated community services within the Royal Borough of Greenwich.We are part of the South East London Urgent Community Response, delivering a rapid response within 2 hours to proactively prevent hospital admission and meet the needs of patients in the community. We are also currently piloting a service working in partnership with the London Ambulance Service community response car.We also provide support, treatment, and monitoring on our virtual ward for up to fourteen days and the team are using digital health care monitoring.The team provides a multi-professional assessment of health and social care needs to support the immediate crisis, identifying specialist assessment and rehabilitation requirements and providing necessary equipment to maintain patients / clients in their own homes.
