The Congleton and Holmes Chapel District Nursing team are looking for a Community Healthcare Assistant who possesses the enthusiasm and positive approach to join a multidisciplinary team with a vision to deliver the right care, at the right time to help improve the lives of our residents.This is an exciting opportunity to join our innovative team to drive forward the expansion and modernisation of community nursing services with commitment to deliver the NHS Long Term Plan.Travel is an essential part of this role. - To contribute and support the delivery of a high standard of effective care, as part of a multidisciplinary team, offering face to face support under the direction of a registered professional
- You will ensure that patients/clients receive safe, individualised and effective care which supports health & wellbeing and promotes self- care where appropriate and possible.
- To provide care to people in their own homes
