Your Opportunity:This is an exciting opportunity to be a member of the Assertive Community Treatment Team (ACT). ACT provides client focused, culturally appropriate, evidenced-based, recovery-oriented care to adults experiencing severe and persistent mental illness, addictions and houselessness. The multi-disciplinary team works primarily with individuals whose illness presents a barrier to engagement in treatment. Services are most often delivered through outreach services in the community, and in partnership and collaboration with clients, families, other AHS providers, inner city agencies, and community programs. Reporting to the Care Manager, the Independent Living Support (ILS) worker assists with the implementation of the client treatment plans and promotion of recovery by providing goal-oriented independent living support and assistance with activities of daily living (i.e. Assistance with budgeting, grocery shopping, accessing the food bank, transit training, attending physician appointments with clients, teaching cooking skills, etc.). The ILS worker must work well with a team approach, have effective communication, observation, and reporting skills, and have the ability to multi-task and prioritize requests. This position has transitioned to Recovery Alberta effective September 1, 2024. You will provide support with preventative health care, health maintenance, quality of life, life skills coaching and connect patients to appropriate community supports. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:Completion of Grade 12 or equivalent. Additional Required Qualifications:Mental Health Aide, Personal Care Aide, or equivalent health-related certificate/diploma and experience required. Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) and Non-Violent Crisis Intervention. Minimum of 2 years of relevant addiction and mental health experience, preferably in a community setting. Demonstrated ability to work well in a team and seek consultation when necessary. Ability to ambulate, climb stairs, and move quickly without limitations in a variety of environments. Preferred Qualifications:Experience and knowledge of the housing first, harm reduction philosophies of care. Demonstrated ability to work well with a range of community and agency stakeholders as well as clients and families.
