- Patient Care: Provide high-quality nursing care to patients in their homes, including administering medications, wound care, and monitoring vital signs.
- Care Coordination: Collaborate with physicians, registered nurses, and other healthcare professionals to develop and implement individualized patient care plans.
- Patient Education: Educate patients and their families on disease management, home care procedures, and health maintenance.
- Documentation: Maintain accurate and detailed patient records, documenting care provided and patient responses.
- Home Health Visits: Conduct regular home health visits, ensuring a safe and comfortable environment for patients.
- Compliance: Adhere to all health and safety regulations, as well as company policies and procedures.
Education and Certification Requirements:
- Valid New Mexico LPN license.
- Home health/hospice experience required.
- Strong clinical skills and knowledge of home health care practices.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
Compensation and Benefits:
- Contract length: 13 weeks
