Contract Duration: 3 monthsAre you a dedicated and experienced Social Worker looking for a rewarding opportunity in Adult Social Care? Walsall Council is seeking a Senior Social Worker to join their team and make a real difference in the community.Role Overview
As a Senior Social Worker, you will work within a Multi-Disciplinary Team (MDT), supporting individuals discharged from the hospital to ensure they receive the right level of care and support.Key Responsibilities:
Complete initial and subsequent reviews of care packages for individuals at home or in short-term placements.
Identify and recommend occupational therapy interventions to promote independence.
Provide assistive technology and signpost to community assets to enhance quality of life.
Assess the need for long-term support and revise support plans when required.
Conduct Mental Capacity Assessments (MCAs) and risk assessments.
Facilitate referrals to extra care housing and in-house provider services.
What We’re Looking For:
Qualified Social Worker registered with Social Work England (SWE).
Proven experience in Adult Social Care and multi-disciplinary team settings.
Strong knowledge of Care Act 2014, MCA 2005, and best practice principles.
Excellent communication and organizational skills.
