The Chipping Surgery is looking for an enthusiastic practice nurse to join our dynamic nursing team. The applicant will ideally have primary care experience including treatment room skills and chronic disease management.The successful candidate will be working with our existing team of a Matron, four practice nurses and six health care assistants. Compassion for patients is key, along with having a positive can-do attitude to continually go that extra mile to deliver an excellent service.Main duties of the jobApplicants will work along side our dedicated nursing team, working 20 hours within our core hours of 8.00am – 6.30pm Monday through to Friday.The successful applicant will have a mixture of treatment room nurse duties and LTC appointments.About usThe successful applicant will be based on our newly refurbished lower ground floor with bright consulting rooms, a minor operations room, meeting room and a well equipped staff room where the team meet for coffee and lunch each day.The Chipping Surgery is very much a team, from reception through to the GPs we pride ourselves on giving excellent care to our patients. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:a. To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the practice managerb. To support the production of the IPC annual statement in conjunction with the practice managerc. To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practiced. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practicee. To develop, implement and embed health promotion and wellbeing programmesf. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeingg. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients healthh. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSFi. As required, to diagnose and manage acute and chronic conditionsj. To prescribe medication for patients in accordance with PGDs and own scope of practicek. To review patient medications to enhance compliancel. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosagesm. To co-ordinate the provision of travel medicine servicesn. To request pathology services and process pathology results as requiredo. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as requiredp. To maintain accurate clinical records in conjunction with extant legislationq. To ensure read codes are used effectively by all members of the nursing teamr. To ensure the appropriate maintenance and use of chronic disease registerss. To develop, implement, embed and participate in well woman clinicst.
