  • Lead and inspire a new team of care professionals, including Care Assistants and Senior Care Assistants.
  • Provide supervision, guidance, and support to ensure our team delivers top-quality care.
  • Develop and implement personalised care plans in consultation with clients, their families, and healthcare professionals.
  • Regularly monitor and review care plans to ensure they meet clients’ evolving needs.
  • Conduct comprehensive assessments of clients’ care needs and develop tailored care plans.
  • Ensure care plans are consistently reviewed and updated.
  • Ensure all care services comply with relevant regulations, policies, and procedures.
  • Conduct regular audits and quality checks to maintain high standards of care.
  • Build and nurture positive relationships with clients, their families, and healthcare professionals.
  • Act as the main point of contact, providing information, support, and reassurance.
  • Offer ongoing training, support, and development opportunities to enhance staff skills and knowledge.
  • Ensure all team members are competent and confident in delivering care services.
  • Maintain accurate and up-to-date records of care plans, assessments, and other relevant information.
  • Utilise appropriate systems and technology for record keeping and reporting.
  • Ensure compliance with health and safety regulations, including risk assessments and infection control.
  • Address any health and safety concerns or incidents promptly.

REQUIREMENTS:

  • Experience as a Care Coordinator or Care Manager with relevant qualifications in social care, nursing, or a related field.
  • NVQ Level 5 is desirable, but candidates with lower-level qualifications are encouraged to apply, with full support for further learning and education provided.
  • Proven experience in managing or coordinating care services, preferably in a homecare or community setting.
  • Strong knowledge of relevant regulations, policies, and procedures in social care.
  • Excellent communication and interpersonal skills, with the ability to build positive relationships.
  • Ability to lead, manage, and motivate a team effectively.
  • Exceptional organisational and time management skills, with the ability to prioritise tasks.
  • Strong problem-solving skills and the ability to make sound decisions.
  • Proficiency in using computer systems and technology for record keeping and reporting.
