HCRG Care Groupis seeking a highly skilled and dedicatedBand 7 SpecialistDiabetes Nurseto join our team atOrmskirk, Lancashire. As a key member of our community healthcare team, you will have the opportunity to make a real impact on the lives of patients living with diabetes.The Diabetes Specialist Nurse service is to promote the health and wellbeing of people with diabetes enabling them to achieve optimum glycaemic control through informed self-management. The successful candidate will provide a link between GP and consultant led specialist teams and will provide education and support both to people with diabetes and those who care for them.This is apermanent full-time role working 37.5 hours per week, Monday – Friday.You will have the opportunity to work alongside a supportive and collaborative team, and to further develop your skills and expertise in diabetes care.If you’re passionate about making a difference and delivering high-quality care, we want to hear from you.Main duties of the jobThe successful candidate will be responsible for:
- Management of own workload, responding to urgent clinical/service needs across locality and liaising with colleagues as required.
- To contribute to specialist protocols and policies for diabetes.
- Assess, plan and implement specialist nursing care programs. This may be for individual patients or for groups of patients.
- Responsible for providing specialist nursing care to patients with complex needs, e.g., those patients requiring insulin therapy.

- A registered general nurse (NMC)
- A V300 Independent Nurses prescriber (evidence: Certificate)
- A minimum of 2 years experience of working in the field of diabetes
- Experience of working in the community
- Full UK driving license and access to vehicle
