The range of assigned tasks will be carried out with the highest standard to support and enable registered nursing staff to provide patient care, which reflects the quality standards, values and behaviours of the University Health Board.Support the delivery of a comprehensive nursing service including chronic disease management, public health nursing and first contact nursing in prison premises and in cell care to coordinate the care needs of patients.Support the Nurse Manager in developing the Nursing Service.The post holder is responsible for the delivery of nursing services to the practice population.They will be supervised and supported by senior nurses within the service, and they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring chronic conditions.The post holder will provide health prevention and screening activities.They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.The role will involve flexible working and travelling as appropriate to the needs of the service in consultation with the post holder and in accordance with Local Service Agreements.Main duties of the jobDUTIES/RESPONSIBILITIES:The post holder will :o Be responsible for maintaining the standards of patient care set by primary care.o Be responsible for ensuring that all policies and procedures are observed.o Be responsible for the support and supervision of new/less experienced staff.o Be responsible for providing accurate, current, comprehensive and concise records concerning the condition of the client.o Liaise with the appropriate Clinical Specialists i.e. GP’s, Practice teams and Clinical Nurse Specialists, in relation to individual patient requirements.o Work in accordance with the Nursing and midwifery Council Code of Conduct.o Demonstrate good organisational skills with responsibility for ensuring efficient and effective use of human and financial resources.About usCwm Taf Morgannwg University Health Board is part of the NHS Wales family.
