The Anne Arundel County Mental Health Agency has an opening for an Adult Aftercare Specialist – Full
Time – On Site in Annapolis MD – Salary range – $50,000 – $58,000 per yearThis position reports to the Clinical Support Specialist Supervisor, is a non-supervisory role functioning
as a care manager for adults requiring behavioral health treatment and related services. Aftercare services
are provided for client treatment, post-hospitalization, or post-detention, RRP or for individuals who need
referral information. The Adult Aftercare Specialist advocates for referred adult clients to help them
attain satisfactory social, economic, mental, or physical wellness, provides referrals, and negotiates on
behalf of the client for receipt of services while remaining current on participating community providers.Qualifications & Abilities:
-Bachelor’s Degree required, Master’s Degree, and License preferred
-Highly organized and meticulous
-Demonstrates ability as an effective care manager
-Knowledge of local and county resources
-Knowledge of Medicaid, Social Security and other entitlements, preferred
-Excellent people skills required
-Excellent verbal and written communication skills
-Demonstrates competency with manual and computerized database entry and record keepingMajor Duties & Responsibilities:
-Provides technical assistance and community referrals for adult service calls, maintains monthly call volume database to provide annual reports and review for the Agency and BHA
-Maintains SOAR consumer servicing data, prepares reports on utilization statistics to SOAR, PATH and the Agency, works with DSS Homeless Coordinator to have data input into HMIS
-Assists SOAR-trained individuals, as necessary, administers database for PATH funds and approves bills sent for SOAR workers
-Assists DOC referrals including scheduling of initial psychiatric appointments and case management intakes for discharging inmates, maintains database of DOC referrals
-Provides administrative oversight support for grants and community partners by reviewing funding requests and billings, attending designated community and provider meetings, reviewing monthly progress notes, maintaining utilization and service reports, assisting with Agency report updates, administering satisfaction surveys for RRP clients, serving on assigned committees and providing database updatesSend resume to [email protected]
