Job description
We are looking for an Clinical Pharmacist to join our existing Pharmacy team. Mosaic PCN services around 60,000 across 3 GP Practices in the Basingstoke area, they are: Crown Heights Medical Centre, Bramblys Grange Medical Practice and Odiham Health Centre. There are also 2 branch sites within our PCN, which are: Old Basing Health Centre and Rooksdown Practice. This role will be based at Brambly Grange Medical Practice, although in the future you maybe asked to work at the other Practices that form part of the PCN.Main duties of the job
- Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
- Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
- General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
- Managing and treating long-term conditions, in line with national and local policies.
- Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols.
- Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
- Meeting the needs of patients presenting for opportunistic wound care.
- Running asthma, COPD and diabetes clinics and reviews.

This includes telephone triage and F2F assessments.Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.Diagnose and manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients.Support the team in dealing with clinical emergencies.Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, nation and practice protocols and within the scope of qualified practice.Recognise, assess, and refer patients presenting with mental health needs.Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health.Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.Support chronic disease clinics, delivering patient care as necessary, referring patients to secondary / highly specialised care advice beyond the scope of normal nursing practice e.g. care treatment plans, making referrals as appropriate (including fast track referrals).Contribute to local and national practice targets clinical remit e.g. QOF, prescribing incentive scheme and national benchmarking.Liaise with external services or agencies to ensure the patient is supported appropriately (vulnerable patients etc.).Support the clinical team with all safeguarding matters for children and vulnerable adults, in accordance with local and national protocols.Understand local policies for substance abuse and addictive behaviour, referring patients appropriatelyManagement of health screening programmes and ensure other team members are aware of health priorities and screening objectives and programmes.Develop, implement and embed health promotion and well-being programmes.
