To this end, NHS Scotland welcomes applications from all sections of society.We are looking for experienced ANPs to join Dundee Enhanced Care at Home Team.Preferred applicants will have experience working at an advanced level with frail elderly patients, with the ability to make professional autonomous clinical decisions through history taking, physical examination and problem solving. The role will involve working autonomously supported by a Lead Advanced Nurse Practitioner and working collaboratively with all members of the multidisciplinary team within community settings.You will assess, diagnose and manage older people with acute care needs living at home and in care homes in Dundee. You will work closely with older people and their families/carers to identify clinical issues quickly and aim to support avoidable deterioration, admission/readmission to inpatient settings and promote and manage good quality end of life care.You will also work with the Discharge Hub to facilitate timely discharge and smooth, efficient and person-centred transitions of care.You will be expected to act as a clinical leader, working in close collaboration with the members of the MDT and will play a key role in the leadership and development of advanced nursing practice within Dundee. You will participate in the delivery of specialist education and training programmes and will work collaboratively with health and social care colleagues across the partnership to deliver high quality evidence-based care.You will also play a key role in enhancing patient experience, assuring clinical governance and supporting the Lead Advanced Nurse Practitioner and Senior Leadership Team in the improvement and continued development of the service.In return we offer the opportunity to work with a dynamic Health & Social Care Partnership and the successful applicants will be supported to continue to develop and demonstrate advanced level practice to maximise the wider opportunities to improve the health and social care outcomes for the population of Dundee.Preferred applicants will be registered nurses, will hold a post graduate diploma, or hold a masters degree in advanced Practice, and be a non medical prescriber.You must have the personal drive, clinical knowledge and commitment to undertake this challenging and rewarding role.
