Heart of Bath Medical Partnership are looking for a full time Advanced Nurse Practitioner. We are a Practice based across 2 sites in the City of Bath. We have a diverse team of clinical and non-clinical staff who work together to provide care for our 26,500 patients.The post holder will be an experienced Nurse Practitioner who, acting within their professional boundaries, will provide assessment, diagnosis, and treatments to acute episodes of minor health conditions as well as minor injuries.The ANP will be expected to see patients autonomously providing face to face, Telephone consultations within the practice and home visits within the patients residence and demonstrate the use of critical thinking in the clinical decision-making process.You will work collaboratively with the general practice team and meet the needs of the patients, supporting the delivery of policy and procedures and providing Nurse leadership as well as promoting self-care to our patients.Main duties of the jobThe ANP will be expected to see patients autonomously providing face to face, Telephone consultations and home visits demonstrating the use of critical thinking in the clinical decision-making process.You will work collaboratively with the general practice team and meet the needs of the patients, supporting the delivery of policy and procedures and providing Nurse leadership as well as promoting self-care to our patients.Main Duties:

  • Assess, diagnose, plan & implement care for patients.
  • Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
  • Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into management plans.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs in accordance to guidelines.
  • Prioritise health problems and intervene appropriately to assist the patient in all situations.
  • Support patients to adopt health promotion strategies and apply principles of self care.
