Hampshire and Isle of Wight Healthcare has exciting opportunity for an experienced CAMHS Community Nurse.Eastleigh CAMHS is a Community CAMHS team within Hampshire CAMHS, and is made up of a wide range of professionals. We offer a supportive and engaging environment, with opportunities for learning constantly offered, regular supervision is in place, and peer support is readily available. We try to focus on wellbeing, have regular team days and time, and value the role of the Nurse highly. Nurses are supported across teams with their CPD, learning opportunities, and this year have had a specific Nurses awayday, which was very well received by those who attended.The role of Community Nurse is to work in partnership with young people and families in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives. They will demonstrate an understanding of clinical interventions relevant the care group to include education, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this. The post holder will work at all times to promote the safety and the well-being of children, young people and their families/carers. The post holder will assess, plan and implement care, and provide specialist nursing advice and carry out specialist nursing procedures. They will provide clinical supervision to staff and students and act as a mentor for student nurses.Main duties of the jobThe post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users by undertaking the following duties:*To undertake assessments of individuals and their families in the designated care group with complex health presentations, including those service users presenting with higher levels of risk. To develop alongside the service user their care plan.*To offer specialist nursing assessment and advice where required.*To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers.
