You will have access to formal academic modules, in house training and regular supervision to progress on your individual continued professional development.Following successful staff development and internal promotion, we have flexible, permanent, fixed term and bank work opportunities within our highly regarded team.The service works in collaboration with Southampton Children’s Hospital and Primary Care settings, delivering acute and ambulatory care in the community, to prevent unnecessary hospital attendance or admission and facilitate early discharge. It’s very rewarding to receive positive feedback from families” CH@H NurseMain duties of the job*Discharge support, acute assessment, nursing diagnosis and care, treatment and referral.*To carry out initial assessment and triage of patients with short term health care needs by telephone or in patient’s homes.*To provide continuity of care for children on caseload and maintain contemporaneous records accordingly.*To discharge patients according to pathways of care*To escalate any changes in the child’s condition appropriately and ensure all records are maintained in accordance with Trust policies.*Participate in the collection of data as required for evaluation of the service and take part in audit as required by the Trust.*The Staff Nurse is responsible for maintaining their own knowledge and competencies in accordance with Trust policies and NMC Code of Conduct.*Provide specialist nursing intervention in relation to symptom management and emotional support for the children and their family.*Administration of medicines including intravenous therapy in accordance with Service/Trust policy*Safeguarding: actively promote the welfare of children and adults*Safely manage high risk areas including lone working, patient handling in challenging environment, and driving.About usHampshire and Isle of Wight Healthcare NHS Foundation Trust is a newly established organisation, bringing together expertise from across mental health, learning disabilities, community, and physical health services.
