About the Department
The Pediatric and Adolescent HIV/AIDS Program, Care2Prevent, in the section of Pediatric Infectious Diseases provides comprehensive treatment and prevention services for people living with HIV and those who are most vulnerable to HIV infection. Job Summary
The Client Care Liaison manages communications and scheduling with Care2Prevent clients to support their care. They provide excellent customer service and will help coordinate with various members of the care team, help maintain appointment records, develop reports on appointment attendance, and triage client communications for action. Through their work, they will also help to develop systems and processes for managing appointments and appointment communications. The Client Care Liaison is an essential member of the Care2Prevent team, interacting with a multidisciplinary team to ensure seamless communication between clients and care providers with an emphasis on client care engagement and appointment adherence.
This at-will position is wholly funded by contractual grant funding which is renewed under provisions set by the grantor of the contract. Employment will be contingent upon the continued receipt of these grant funds and satisfactory job performance.
Responsibilities

Support client appointment scheduling and develop reports to track appointment attendance.

Manage appointment reminder communications and follow-up.

Interface with the clinic referrals desk to support client appointment scheduling in the C2P clinic.

Help support the team in completing new client intake such as recording eligibility documentation and collecting contact and demographic information.

Oversee the C2P clinic schedule including actively participating in pre-clinic huddles to provide updates on clinic schedule changes.

May initiate correspondence with clients and other written materials under own signature or for signature of the leadership team.

Triages emails and telephone calls for action. Resolves mostly routine and some complex inquiries.

Prepare special reports and summaries, assists in managing daily operation of the office, and may assist in developing administrative goals and policies.

Performs other related work as needed.
Minimum Qualifications
Education:
Minimum requirements include vocational training, apprenticeships or the equivalent experience in related field (not typically required to have a four-year degree).

Work Experience:
Minimum requirements include knowledge and skills developed through 2-5 years of work experience in a related job discipline.

Preferred Qualifications
Education:

GED or High School Diploma.
Experience:
-
1-2 years of experience working in HIV or sexual health.

1-2 years of experience working with sexual, gender, and racial minority populations LGBTQ+, minority, and marginalized populations.
