To learn more about CMH, please visit our website at .Job Description:Salary Range: $52.13 – $62.06Reporting to the Manager of Medicine and Rehab, the Clinical Coordinator provides leadership and operational supervision within the Medicine Program. The Clinical Coordinator facilitates day to day operations collaborating with nursing, allied health professionals, physicians and management.The Clinical Coordinator will work in partnership with the Manager to mentor and coach staff responsible for patient care, work with Physicians and other health care professionals across the hospital to ensure an interdisciplinary and efficient approach to patient care is achieved. Assists in operations, planning and implementation, to achieve corporate and program goals with emphasis on quality and person centered care.The Clinical Coordinator promotes a safe patient focused environment through the implementation of evidenced-based practice, and continuous quality improvement including developing, implementing and monitoring performance indicators. Works with the Manager to execute goals and objectives for the program/service areas, is responsible for patient flow, patient experience and assisting in program development through regular staff interactions and communications.Participates with all health care team members in formulating and implementing policies, procedures and standards necessary to support excellence in clinical services. Critically analyzes and promotes evidence based practice.The Clinical Coordinator is responsible for compliance with all legislative and regulatory requirements related to practice standards and patient safety, and ensures the implementation of improvement strategies as required. Manages quality initiatives, works to mitigate risk including review and follow up to unusual occurrences, working closely with staff and in consultation with the Manager. Works in partnership with the lead for Quality and Patient Safety to foster an environment of stewardship. Attends Clinical Service Department meetings to ensure understanding and facilitate application of the Hospital’s vision, mission, values, goals and objectives.Posted hours does not constitute a guarantee that shifts will not be subject to change.Qualifications:
- Regulated health care professional holding current registration with a regulated College in Ontario
- BScN or undergraduate degree required
- Master’s degree preferred or in progress with a commitment to complete within five (5) years
- Minimum of 3-5 years clinical experience, acute medicine in the hospital setting
- Minimum of 3 – 5 years formal leadership experience
- Computer proficiency (Microsoft Suite including Word, Excel, PowerPoint, Outlook)
- Evidence of ongoing education and professional development related to professional practice
- Evidence of leadership education and training, including Quality Improvement and Crucial Conversations or a willingness to complete in the first year
- Demonstrated leadership skills & abilities
- Regular attendance required
