Purpose of the PostThe clinical nurse manager 2 in Dementia care will assess, plan, implement and evaluate care delivered to patients with Dementia. The post holder must exhibit the highest professional and ethical standards within the model of nursing care practiced in the relevant care setting.Principal Duties and ResponsibilitiesProfessional responsibilities:· Practice in accordance with relevant legislation and with regard to The Scope of Nursing & Midwifery Practice Framework (Nursing and Midwifery Board of Ireland, 2015) and the Code of Professional Conduct and Ethics for Registered Nurses and Registered Midwives (Nursing and Midwifery Board of Ireland, 2014).· Maintain a high standard of professional behaviour and be professionally accountable for actions/omissions. Take measures to develop and maintain the competences required for professional practice.· Adhere to the Nursing & Midwifery values of Care, Compassion and Commitment (DoH, 2016).· Adhere to national, regional and local HSE PPPGs.Clinical Responsibilities:· Be responsible for the co-ordination, assessment, planning, implementation and review of care for people with dementia.· Encourage evidence based practice, using a care planning approach to nursing care.· Increase dementia and delirium awareness among health care professionals who provide direct (nurses, HCA, AHPs, doctors) and indirect care staff (porters, catering, cleaning).· Ensure that patients receive dementia timely diagnosis and interventions· Ensure care delivered is in according toi. National dementia guidelinesii. Professional best practice guidelinesiii. Local policies protocols and guidelinesiv. Current legislation as appropriate (Assisted Decision- Making (capacity) Act 2015.· Improve clinical outcomes through reducing risk of hospital associated decline with dementia attuned withI. mobilization,II. nutritionIII. Cognitive engagement strategies.· Collaborate with service users, family, carers and other staff in treatment / care planning and in the provision of support and advice.· Communicate verbally and / or in writing results of assessments, treatment / care programmes and recommendations to the team and relevant others in accordance with service policy.· Participate in multidisciplinary team meetings to improve patient outcomes.· Plan discharge or transition of the service user between services as appropriate, through liaison with residential, Community and PCCC services including Public Health Nurses, Integrated Care Manager, Nursing Homes etc.· Contribute to local Key Performances Indications (KPIs) in line with local and South West Hospital Group best practice for Dementia and Delirium care.· Contribute to the development of patient centred care plans and care pathways.· Contribute to the development and maintenance of nursing standards, protocols and guidelines consistent with the highest standards of patient care.· Lead quality improvement projects to improve care for older people· Participate in national and local audit and policy development.· Devise and implement Health Promotion Programmes for service users as relevant to the post.· Develop links and participate in regional and national dementia networks (National Dementia Office) and build collaborations with third sector organisations e.g Memory Cafes, Alzheimer’s Society, National Hospice, local Age Friendly groups.Health and Safety· Adhere to health and safety guidelines and updated COVID-19 Guidelines and Pathways.· Observe report and take appropriate action on any matter which may be detrimental to service user care or well-being / may be inhibiting the efficient provision of care.· Ensure completion of incident / near miss forms.· Participate in development of PPPG to maintain safety of people with dementia.· Have a working knowledge of the Health Information and Quality Authority (HIQA) Standards as they apply to the role for example, Standards for Healthcare, National Standards for the Prevention and Control of Healthcare Associated Infections, Hygiene Standards etc and comply with associated HSE protocols for implementing and maintaining these standards.Education and training:· Provide dementia/delirium awareness education, in terms of knowledge, skills, and attitudes for all healthcare staff on the importance of dementia friendly and person-centred approaches to care.· Contribute to the roll out of education training on site:Facilitator for the Implementation of National Clinical Guideline No. 21Facilitator for Enhancing and Enabling Quality of Care with the Person with Dementia-Acute Care Programme. (4 hour training).Facilitator for Enhancing and Enabling Well Being for the Person Living with Dementia Programme, (2 Day training).· Maintain clinical competence in patient management within Delirium, Cognitive Impairment and Dementia, keeping up-to-date with relevant research to ensure the implementation of evidence based practice.· Provide the patient, family and/or carer with appropriate information and other supportive interventions to increase their knowledge, skill and confidence in managing their dementia condition.· Provide supervision and assist in the development of knowledge, skills and attitudes of staff and assigned students.· Develop and maintain links with Regional Centres for Nursing & Midwifery Education (RCNMEs), the Nursing & Midwifery Planning and Development Units (NMPDUs) and relevant third level Higher Education Institutes (HEIs) in the design, development, and delivery of educational programmes in dementia care.Personal and administrative:· Contribute to the service planning process and identify changing needs and opportunities to improve services.· Co-operate in managing all resources – including nursing and non-nursing staff within an agreed budget.· Maintain a data base of your own case load, training and education.The above Job Description is not intended to be a comprehensive list of all duties involved and consequently, the post holder may be required to perform other duties as appropriate to the post which may be assigned to him/her from time to time and to contribute to the development of the post while in office.The reform programme outlined for the Health Services may impact on this role and as structures change the job description may be reviewed.Informal EnquiriesWe welcome enquiries about the role.Contact Ms Teresa Bohane– ANP Dementia Service, 02750133- for further information about the roleContact Eimear O’Sullivan- People Resourcing, – for enquiries relating to the recruitment process
