Community Psychiatric Nurse – Newport Older Adult CMHTJoin our dynamic and supportive Older Adult Community Mental Health Team (CMHT) in Newport, where you will play a pivotal role in delivering high-quality mental health care to older adults with complex mental health needs. As part of our multidisciplinary team, you will provide holistic, person-centered care, including comprehensive assessments, care planning, risk management, and therapeutic interventions.In this role, you will work collaboratively with patients, families, carers, and other professionals to ensure continuity of care and positive outcomes. If you are seeking a rewarding role where you can make a real difference in the lives of patients and their families, we would love to hear from you.Main duties of the jobThe Community Psychiatric Nurse will work within their code of practice to take responsibility for their allocated caseload to ensure assessments of health and social needs. This will include formulation, planning and evaluation of care which include regular reviews and update of risk assessments documentation and care planning.
