You will be evaluating evidence-based medicine to support your recommendations and to develop improvement strategies for drug utilization. You will also be accountable for delivering on assigned clinical pharmacy projects. You will collaborate with the pharmacy leadership team, various pharmacy staff and external stakeholders on pharmacy projects. You will be an active participant in relevant inter-professional hospital committees. As required, you will travel between the three hospital sites.Key Qualifications:
- Baccalaureate Degree in Pharmacy or Pharm D.
- Licensed in Ontario to practice pharmacy (Part A of Register)
- Completion of hospital pharmacy residency or equivalent experience is preferred; equivalent experience is interpreted as three years of experience in a broad range of pharmacy operational, clinical and/or administrative pharmacy practice
- Minimum 3 years of recent experience in hospital pharmacy practice required
- Completion of leadership training is an asset
- Project management training or experience working on projects is an asset
- Active membership in the Canadian Society of Hospital Pharmacists (CSHP) is an asset
- Strong leadership, interpersonal, problem solving, organizational and communication skills
- Experience in providing exceptional patient centered care
- Experience in hospital pharmacy systems and distribution systems are an asset.
- Past experience or a keen interest in teaching and research activities is an asset
- Relevant clinical experience preferred
- Ability to work in a fast-paced environment both independently and cooperatively with the team
- Demonstrated self-directed learning and continuing education
- Positive attitude, flexibility to adapt to changing organization needs and accountability
Project responsibilities include but are not limited to:
- Act as a mentor and resource for staff
- Review and participate in enhancing comprehensive pharmacy clinical programs through the projects assigned
- Assist Clinical Manager of Pharmacy in standardizing pharmacy policies and procedures in accordance with hospital initiatives, professional practice standards and provincial and federal laws.,
- Work in conjunction with the Pharmacy Leadership Team and the Clinical Manager to prioritize, coordinate and integrate workload
- Develop project work plans in coordination with appropriate internal and external stakeholders to establish project scope, deliverables, accountabilities, schedules, timelines, resources, monitoring, and post project audit/evaluation
- Effectively communicate project expectations to team members and stakeholders in a timely and clear fashion
- Liaise with the clinical manager, leadership team and project stakeholders on an ongoing basis
- Plan, schedule and manage project timelines and milestones
- Collaborate with physicians, nurses, pharmacists, pharmacy technicians and allied health team members regarding project progress and accountabilities
- Manage change associated with the assigned projects
- Will be expected to support other projects or duties as assigned
Clinical responsibilities include but are not limited to:
- Provision of comprehensive pharmaceutical care to patients
- Clinical care of patients in comprehensive clinical programs including warfarin dosing, IV to PO, aminoglycoside and vancomycin dosing.
- Use of Pharmacists’ clinical scope of practice as per THP’s Policy and Procedure to optimize patient care
- In collaboration with other clinical pharmacists deliver appropriate teaching to pharmacy residents and pharmacy students
- Participate in pharmacy case presentations, Pharmacy Education Rounds, journal clubs, case discussions and research projects
- Drug distribution responsibilities.
- Collaborate with physicians, nurses and allied health team members to provide optimized patient care
- Other duties as assigned including the ability to potentially work at all Trillium Health Partners sites
- Must be able to provide coverage on various clinical units as needed
