Come and make a difference by joining our innovative Intermediate Care Reablement Team. The role of Occupational Therapy is essential in supporting people to transition back to their home environment, by focusing on what they want to, need to, have to and most importantly LIKE to do to ensure they life have purpose value and meaning.The team consists of a Discharge Co-ordinator, OTs, Physios, Reablement Support Workers & Adult Social Care Workers all working with a shared vision of putting the person at the centre of their care. The role is based within a care home team, which is walking distance from the centre of Taunton and the ethos is to enable people to be as independent as possible. We see people not only for their ongoing rehabilitations and support needs once discharged from the Acute hospital, but also through to the next stage of their journey into their home environment. This role works in collaboration with ongoing services to enable a person to maximise their potential in all aspects of occupations that are important to them.We have a fantastic Band 6 full-time opportunity. You can expect to have regular supervision, annual appraisal and Inservice Training.Main duties of the jobIn this post, you will be supported to:Develop, use and be creative with your OT skills to work holistically and look deeper than the original diagnosisHave the freedom and support to act in patients best interests & what matters to youConnect with, assess and treat patients who may have diverse presentations and complex physical and psychological conditionsWork with patients, to set personalised goals and individual treatment programmesWork with patients, their families and the MDT to plan and implement dischargeOrganise, prioritise and plan your own caseloadClinically supervise other members of the teamTake a role in the development of the Intermediate Care Service within SomersetYour professional supervision, includes support from the team leader who is an experienced AHP therapist, working with you in intermediate care. Links with other occupational therapists within the intermediate care service will be supported.The Occupational Therapy leads and Occupational Therapists across all agencies in Somerset are working together to implement our OT strategy with the aims of better experience for staff, better experience of care, better outcomes and values.The post holder will be required to work their hours over 7 days, including out of hours, weekends and bank holidays, to meet the needs of the service.
