  • Patient Care: Provide high-quality nursing care to patients in their homes, including administering medications, wound care, and monitoring vital signs.
  • Care Coordination: Collaborate with physicians, registered nurses, and other healthcare professionals to develop and implement individualized patient care plans.
  • Patient Education: Educate patients and their families on disease management, home care procedures, and health maintenance.
  • Documentation: Maintain accurate and detailed patient records, documenting care provided and patient responses.
  • Home Health Visits: Conduct regular home health visits, ensuring a safe and comfortable environment for patients.
  • Compliance: Adhere to all health and safety regulations, as well as company policies and procedures.

Education and Certification Requirements:

  • Valid New Mexico LPN license.
  • Prefer candidates with home health and hospice experience.
  • Strong clinical skills and knowledge of home health care practices.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
