St Helens North Primary Care Network are looking to recruit to a new Advanced Nurse Practitioner who is a caring, compassionate and experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within general practice.The successful applicant will work within our friendly Primary Care Network multidisciplinary healthcare team alongside another PCN ANP in the same role and 2 Pro-active Frailty ANPs, and in partnership with the six practices that make up the Primary Care Network.We are unable to accept application from applicants who require Visa Sponsorship.No recruitment agences please.Main duties of the jobWithin our PCN, we have recently created an Urgent Hub model where all six PCN practices will be able to book in to your acute hub clinic for same-day patients. Using your clinical expertise, the primary focus of the role is to work as an autonomous practitioner providing high-quality and timely patient-centred care to assess patients who present with a broad range of situations and conditions.The post holder will be responsible for the assessment, diagnosis, treatment, referral, and discharge of a diverse range of patients across the age continuum with acute illnesses; independently within their level of competence but with GP support when needed.The second part of the role will be time spent in individual practices, seeing their patients only to provide care for both acute issues and long-standing conditions depending on practitioner competencies. Depending on a successful applicants background/clinical interests there is the possibility to work with practices to offer specific clinics/services in this time instead.Other aspects of the role will be working alongside/covering other ANP colleague in the team who covers care home, working with the Clinical Director and Clinical Lead to help develop the urgent hub service and also other services for the PCN, offering education and mentoring to more junior nursing colleagues or students in the PCN.The successful candidate will have an advanced practice qualification & be a non-medical prescriber.About usThe post holder will be working as part of the St Helens North Primary Care Network.
