Healthier West Wirral are looking for A Physician Associate who is passionate about Primary Care to join our well established Physician Associate team. Your role will require you to participate in all aspects of patient care through knowledge gained from your PA training. This is a hybrid role that will see you working in Practice but also in the community as part of our Home Visiting Service.

Physician Associates will work within their scope of clinical practice and demonstrate safe clinical decision making and care for patients within the GP practice in consultation with the supervising GP.

To be considered for this role you will have completed the Physician Associate Postgraduate Diploma/Masters from an accredited UK university and passed the Physician Associate National Examination.

Job responsibilities

Healthier West Wirral PCN supports a population of over 70,000 patients across 6 different Practices including Marine Lake Medical Practice, Heswall and Pensby Group Practice, Commonfield Road Surgery, Upton Group Practice, West Wirral Group Practice and Greasby Group Practice.

Key role requirements Physician Associates will work within their scope of clinical practice and demonstrate safe clinical decision making and care for patients within the GP practice in consultation with the supervising GP:

  • Provide first point of contact care for patients presenting with undifferentiated, undiagnosed problems by utilising history taking, physical examinations and clinical decision-making skills to establish a working diagnosis and management plan in partnership with the patient (and carers as applicable).
  • Identify, signpost, or refer patients at risk of developing long-term conditions, preventing adverse effects on the patients health
  • Support the management of patients conditions through offering specialised clinics following appropriate training including (but not limited to) family planning, baby checks, COPD, asthma, diabetes, and anticoagulation
  • Provide health / disease promotion and prevention advise alongside analysing and actioning diagnostic test results
  • Examine, assess and diagnose patients and provide clinical care/ management as required
  • Triage patients and provide the necessary treatment during home visits
  • Maintain accurate clinical records including read codes in conjunction with legislation
  • Ensure continuity of care, arranging follow-up consultations or reviews as necessary
  • Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes
  • Recognise, assess and refer patients presenting with mental health needs

Support the clinical team with all safeguarding matters, in accordance with local and national policies.

In addition to the primary responsibilities, the physician associate may be requested to support PCN initiatives:

  • Support audit programmes, undertaking audits when necessary
  • Participate in local initiatives to enhance service delivery and patient care
  • Support and participate in shared learning within the practice.
  • Take personal responsibility for own learning and development, including the requirement to maintain currency, achieving all targets set in their Personal Development Plan (PDP).
  • Travel between PCN practices when required to support with annual leave.

Schedule: Monday- Friday, with the ability to work evenings or weekends on rare occasions

*Please note, this role will require travel between practices within our locality site and a full, current UK driving and use of a car will be required.
