Vancouver Coastal Health is looking for a Rehabilitation Assistant to join the Personalized Support and Stabilization team supporting Robert and Lily Lee Family Community Health Centre in Vancouver, BC. Apply today to join our team!
As a Rehabilitation Assistant with VCH you will make your impact by:
- Function under the operational supervision of the Manager or Team Leader and the work direction of a licensed designated occupational therapist or physiotherapist to work as a member of the health care team.
- Provide assigned physical / occupational therapy therapeutic interventions;
- Assist in modification of OT/ PT care plans and facilitates transition to community programs in order to enable clients to maximize potential for independence.
- Provide functional direction to family, report relevant observations of client’s condition in writing or by utilizing basic computer literacy, manages equipment safety and inventory.
About Personalized Support and Stabilization (PSS+):
Personalized Support and Stabilization is a community team of interdisciplinary professionals working together to provide intensive short term support for people with complex health conditions to safely transition, from hospital settings back to their homes in the community.
Qualifications
Education & Experience
- Completion of a recognized Diploma level Rehabilitation Assistant Program with Occupational and Physical therapy components.
- One (1) year’s recent, related experience working with target population.
- Valid BC Drivers License. Local area travel requires the use of a personal vehicle for which mileage will be reimbursed.

- Ability to communicate effectively both verbally and in writing.
- Computer literacy skills.
- Ability to work collaboratively with others.
- Physical ability to carry out the duties of the position.
- Ability to organize and prioritize work.
- Ability to observe and recognize changes in clients.
- Ability to analyze and solve problems.
- Ability to establish and maintain rapport with clients.
- Ability to motivate clients and to encourage progression through therapeutic goals.
- Demonstrated ability to provide self-care instruction and education.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Successful applicants may be required to complete a Criminal Records Review Check.
