How you’ll make a difference:As a Social Care Practitioner, you will make a real difference with vulnerable adults in our community by assisting them in their time of need, providing support and promoting their independence.What you will be doing:
- Working alongside social workers, you will help to build relationships with people who have complex needs and assist with various practical tasks.
- To help safeguard our vulnerable adults, you will be completing Care Act assessments and reviews with the oversight of the Senior Practitioner.
- With the support of the Senior Practitioner, you will be completing S42 enquiries.
- You’ll refer issues if needed in accordance with safeguarding procedures to protect vulnerable people. This will require you to maintain confidential reports to professional and council guidelines.
- You’ll be communicating, providing advice and guidance to service users and partner agencies.
What we need from you:
- We require you to have experience in a relevant area of statuary or voluntary sector work in health and social care.
- You will demonstrate knowledge of relevant legislation, regulations and codes of practice; processes and procedures; support mechanisms and specialist equipment.
- You will be able to gather information from a variety of sources and follow the assessment process.
- Ideally, you will have relevant qualification e.g. NVQ Level 3 in health or social care field.
What you need to know
- This role is hybrid, with a mixture of working at our office at Badminton Road, Yate, in the community and occasionally from home.
- Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post.
